Pre Referral Checklists and Guidelines

When making a referral please:

a) Follow Pre referral guidelines        b)Use either Individual, group or teacher referral checklist;   

c) Complete the Referral form.

At the referral meeting: 

a) Liaison RTLB will Prioritise according to the prioritisation quadrant

b) An RTLB will be allocated the referral and will set up the

c) Entry Meeting / Initial working contract;    

d) Use the service delivery framework process to support the referral
